Thank you for signing up for your new service(s) at CenturyLink. You have earned a Visa Gift Card. Complete your registration and we will send it right out to you.

How it works:

Read and agree to the Terms and Conditions listed below. Click on the I Agree button. You will be taken to another page where you can complete your registration and answer a few questions about your household use of digital entertainment and communication products. When you have completed the registration click on the Save button and you are done. Be sure to click Save when you are through or your registration will not be recorded. Your Visa Gift Card will be shipped in about 6 - 8 weeks.

Rules of Participation

To receive the Visa Gift Card, the purchaser of the CenturyLink Services must submit his/her request personally online at no later than 11:59:59 p.m. CDT on May 31, 2010. Online, you will be required to submit all information requested, including your full legal company name, mailing address (no PO boxes, please), telephone number (including area code), email address, and the CenturyLink account number from the qualifying service purchased. One Visa card per business. All participants must have a valid email address. Invalid or incorrect information will result in a Visa card not being issued. CenturyLink, Peter A. Mayer Advertising, and Invenu, LLC employees are not eligible. Please allow 6-8 weeks from submission of request for shipment of the Visa Gift Card. Visa Gift Card is issued by the First National Bank of Omaha pursuant to a license from Visa U.S.A. Inc. Sponsor is not responsible for Visa Gift Card requests that are illegible or incomplete. Sponsor reserves the right to audit any Visa Gift Card request, and/or to nullify any Visa Gift Card request that cannot be substantiated. No responsibility or liability is assumed by Sponsor for technical problems or technical malfunction arising in connection with any of the following occurrences which may affect the operation of the Website: hardware or software errors; faulty computer, telephone, cable, satellite, network, electronic, wireless or Internet connectivity or other online communication problems; errors or limitations of any Internet service providers, servers, hosts or providers; garbled, jumbled or faulty data transmissions; failure of any e-mail transmissions to be sent or received; lost, late, delayed or intercepted e-mail transmissions; inaccessibility of the Website in whole or in part for any reason; traffic congestion on the Internet or the Website; unauthorized human or non-human intervention of the operation of the Offer, including without limitation, unauthorized tampering.

CenturyLink reserves the right to alter or discontinue this program at any time without notice. Participation in the Program constitutes the Participant's full and unconditional agreement to these Terms and Conditions.

Offer is subject to all federal, state and local laws. Void where prohibited.
